League Officers

As a follow up to the new form. Please note that it actually requires 5 signatures 4 of which must be on the form when it leaves my office. I have laid out the procedures to follow below and who is responsible. BTW, we are Region 111.

• TSSAS - The way it is lined out, the State Registrar signs, the top of the first page, Part A to verify that this is a registered player (I may have to verify with the League Registrars if I do not have the player in my data base).
• Team. - The Coach, Manager or Referee now signs the first line of the second page, Part B where it says Signature of Coach, Manager or Referee.
• League Rep - Someone from your league signs where it says Signature of League Verification Officer.
• Player – Must sign bottom of Page 1, Part A.
• USASA – Signs off in the last signature line
All of these signatures need to be gathered before the form gets to me with the exception of the USASA one. I will then do my thing and send the package off to USASA who in turn will forward it on to K&K. This is an extra step and I have no idea why it has been introduced.

I would suggest that each of your leagues needs to nominate one specific to person to sign off so that the injured parties have some procedures to follow at your end. This is entirely up to you but it does have to be someone with authority and preferably the same person.

Things will become clearer as we get into using this new form. As with the last form, do NOT leave any blanks as I will just send it back to you. The most important thing is the 90 day statute of limitations so even if the player does not yet have any invoices, the form should be completed and sent in to me immediately so that we can get the claim registered.
